Job Summary

Intention Healthcare is seeking an experienced and mission driven Medical Director to oversee and guide the Intention Provider team in delivering home‑based primary care across Massachusetts. This role ensures the delivery of high‑quality clinical services through direct oversight of provider performance, documentation standards, quality initiatives, regulatory compliance, and clinical best practices. The Medical Director also serves as the primary clinical resource for the providers, offering mentorship and support for complex cases while fostering a culture of excellence, collaboration, and continuous improvement. The Medical Director will provide direct clinical care under a reduced patient panel.

Responsibilities

Clinical Leadership and Oversight

  • Provide clinical supervision, mentoring and ongoing support to Intention providers across Massachusetts
  • Serve as the primary point of contact for clinical questions, diagnostic guidance, and case consultations
  • Ensure adherence to evidence based clinical standards, treatment protocols and organizational policies
  • Conduct periodic ride‑alongs, case audits and chart reviews to monitor quality and consistency of clinical care

Quality and Performance Management

  • Oversee provider performance metrics, including productivity, patient outcomes, documentation timeliness and quality, and safety standards
  • Lead quality improvement initiatives aimed at clinical effectiveness, patient satisfaction and decreasing utilization rates
  • Develop and implement workflows, best practices and quality benchmarks to optimize operational efficiency
  • Provide feedback, coaching and performance evaluations for providers including individualized improvement plans when needed
  • Ensure closed‑loop clinical communication occurs with external clients, partners and care team members

Program Development and Collaboration

  • Partner with multidisciplinary teams including physicians, PAs, NPs, specialists, behavioral health providers, and care coordinators to support integrated care delivery
  • Participate in strategic planning for clinical service expansion, program design and innovation in home based care
  • Support onboarding, training and continuing education for Intention Providers

Clinical Practice

  • Conduct comprehensive health assessments during home visits, including physical exams, medication reconciliation, and review of medical histories
  • Diagnose medical conditions, develop individualized care plans, and prescribe medications as appropriate
  • Educate patients and families on disease management, lifestyle changes, medication adherence and preventive care
  • Maintain detailed and accurate electronic health records (EHR) of patient assessments, treatments, and follow‑ups

Qualifications

  • Medical Doctor (MD) or Doctor of Osteopathic (DO) degree from an accredited United States medical school
  • Successful completion of residency in internal medicine or family medicine at an accredited healthcare institution
  • Current Massachusetts license and prescriptive authority
  • Board Certification in Internal Medicine or Family Medicine
  • Geriatrics training or palliative care experience is preferred but not required
  • Minimum of 5‑7 years of clinical experience, preferably in home‑based care, primary care, geriatrics, internal medicine or complex disease management
  • Demonstrated leadership experience
  • Proficiency with EMR/EHR systems and documentation standards

Compensation

All‑in compensation $250‑350K comprised of base and performance incentives. Potential to purchase shares in company available for tenured providers.

As a Primary Care Nurse Practitioner, you will provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic, as directed by physician or mid-level provider. You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.

Core Responsibilities

  • Actively diagnoses and treats our veterans under the direction and responsibility of a supervising physician.
  • Examines patient for symptoms of organic or congenital disorders.
  • Develops and implements patient management plans and assists in provision of continuity of care.
  • Orders and performs diagnostic tests, such as x-ray, electrocardiogram, laboratory tests, etc. and interprets test results for deviations from normal and provides patient notification and follow up care.
  • Counsel patients on the use of prescription medications, educates patients, assesses mental health issues, and provides routine health maintenance.
  • Evaluates patients’ records from medical providers outside the VA and works with these patients utilizing rules set forth by the VA for co-managed care.
  • Completes any and all clinical reminders due at the time of each patient visit.
  • Completes documentation of the medical record within 24 hours of a patient encounter.
  • Complies with all VA guidelines in regard to appropriate and timely response to all patient requests, alerts and notifications, consults, orders, lab results, and diagnostic studies.
  • Complies with the VA formulary process and consult protocols.
  • Complies with all VA guidelines in regard to appropriate and timely clinical documentation, including the completion of all encounters by close of business.
  • Complies with all VA and company training requirements.
  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance, and other related items.
  • Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.
  • Must provide excellent customer service to each veteran, both in person and over the phone, as well as to fellow colleagues and clinic visitors.
  • Participate in the clinic’s outreach events to help support the clinic’s enrollment initiatives.
  • Support patient enrollment and retention by providing guidance, when necessary, regarding scheduling, follow-up visits or nurse visits.
  • Embrace and support new initiatives, whether clinical or operational.

Qualifications

  • Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioner (MSN required), including preceptorship.
  • Certification by applicable professional organization.
  • Minimum one-year clinical experience (three years preferred) as a CRNP within the last 4 years in a related primary care or ambulatory care setting; government healthcare environment preferred. Specific requirements could vary based on individual VA contract.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.
  • Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract.

Position Summary

The Regional Medical Director (RMD), Primary Care (PC) has direct responsibility, in conjunction with leadership personnel within MultiCare Medical Partners (MMP), for the operational, clinical, and financial performance for a service line or group of related care lines within a region that make up a clinical division. In partnership with the Service Line Assistant Vice President (AVP), the RMD provides administrative medical leadership for clinical and business planning, expansion of growth and access, clinical performance, and quality outcomes. The RMD works to manage the functions of Primary Care to ensure performance to strategic and operational objectives delineated by MMP and MultiCare Health System (MHS) leadership. The RMD works collaboratively with MHS and MMP leadership, providing supervision and oversight of the Site Medical Managers (SMMs), physicians and advanced practice providers in Primary Care.

Principle Accountabilities:

  • Positively contributes to organizational culture, leading in alignment with organizational mission, vision, and values.
  • The RMD demonstrates operational excellence in dyad partnership with the Service Line AVP in the areas of fiscal access to care, patient experience, quality improvement, clinical outcomes, and employee and physician/APP engagement. The RMD is responsible for leading initiatives in care delivery, clinical quality, and performance improvement, to meet organizational objectives.
  • Holds providers and other caregivers accountable to performance expectations and goals, serving to support escalations as needed from the Site Medical Manager (SMM).

Care Line Specific Responsibilities

For clarification purposes, in the role as Regional Medical Director, it is expected that the Physician will attend meetings to discuss operational issues, planning or execution of initiatives to enhance performance, safety or quality initiatives which are specific to his/her administrative role, where such attendance shall be compensated at the rate set forth above. It is assumed that the physician, as a Medical Staff member, would attend relevant Medical Staff meetings that relate to his/her provision of professional services at Hospital regardless of whether or not Physician served as Regional Medical Director; consequently, the Medical Director shall not be compensated for routine attendance at such meetings. Medical Staff meetings that will not be routinely compensated for under this Agreement include meetings such as department meetings, Special Peer Review meetings, QRM Committee, Medical Executive Committee, Credentials Committee, General Staff meetings, Medical Staff townhalls, and official Board of Directors meetings and subcommittee meetings. A list of activities that generally qualify, or do not qualify, for Medical Director payment is included below:

  • Clinical protocol/evidence -based care development.

  • Routine participation in meetings (department meetings, department division meetings, Medical Executive Committee, General Medical Staff, clinic/practice meetings) where attendance would be expected for any practicing physician. The exception to this is if the Site Medical Manager is, as part of their role, preparing information for the meeting or presenting information for such meeting.
  • Continuing Medical Education (CME) activities, including CME conference attendance and self-directed professional education reading, unless activity is explicitly requested and approved in advance.
  • Local, state, and national medical society meetings
  • Clinical activities, unless participating in a mentoring/proctoring role specifically related to Site Medical Manager duties.
  • Research/academic activities, unless specifically related to SMM goals and duties, and pre-approved by Administrator with oversight accountability.

REQUIREMENTS

  • Graduate of an accredited medical school (MD or DO).
  • Licensure to practice medicine in Washington State as a Physician.
  • Board certification or similar accreditation in Internal Medicine, Family Medicine or Pediatrics.
  • Two (2) years of clinical practice experience preferred.
  • Formal leadership training and experience preferred.

  • Registered or Certified Medical Assistant with six months of related work experience (examples of related fields include military medic, emergency medical technicians, Nurse’s Aide, physical therapy and nurse technicians, and certified athletic trainers).
  • Medical Assistant credentials must be obtained from in-person proctored exams from the following certifying bodies: NHA, AAMA or AMT. In-person proctored credentials from other certifying bodies may be accepted upon approval from Human Resources.
    • Graduate of an accredited nursing program (such as RN/LPN), or comparable allied health training program with a minimum of one year of relevant experience.
    • Ability to show proof of a Medical Assistant certification/registration with successful completion of certification exam (online proctored exams are not sufficient to meet the credential requirement) within six months of hire date (or within a shorter time frame if noted by hiring manager).
  • Basic Life Support certification (Online Basic Life Support certifications, those without a skills assessment component, are not sufficient to meet the Basic Life Support requirements).
