As a Home Health Physical Therapist Assistant, you will:

  • Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
  • Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
  • Document client records and submit clinical documentation to the office on time each week.
  • Attend educational meetings and coordinate department in-services.
  • Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
  • Apply concepts of infection control and universal precautions in performing patient care activities.

POSITION SUMMARY
The Licensed Group Facilitator is a member of a multi-disciplinary treatment team and is responsible for the development, implementation and management of curriculum and treatment of youth referred to the Child and Adolescent Group Services (CAGS).

RESPONSIBILITIES INCLUDE
  1. Responsible for documentation including, but not limited to:
    • Develop and update treatment plans for all clients.
    • Document results of treatment plan reviews for all clients on a semi-annual basis or at the request of parent.
    • Responsible for daily/progress notes for therapeutic service hours delivered, as shared with other facilitators within the groups.
    • Responsible for weekly summaries of assigned youth within CAGS.
    • Complete appropriate summary report when a client ends participation in CAGS (discharge, transfer or closing summary).
    • Report all incidents and documents the same day to the CAGS Clinical Supervisor.
  2. Administer appropriate therapeutic groups to youth within various clinical topics as directed via curriculum based on individual client treatment needs.
  3. Provide required amount of delivery hours per week for this position.
  4. Responsible for group/team leading and clinical planning of therapeutic groups and individual clients involved in CAGS. 
  5. Supervise client activities and is accountable for whereabouts and safety of clients during CAGS time.
  6. Protect clients from physical and emotional harm emanating from themselves, others, or the environment including use of the Therapeutic Crisis Model of Intervention, utilizing physical restraints as necessary.
  7. Assist in crisis intervention, problem resolution and process discussions with clients on an assigned and an as needed basis.
  8. Responsible for the planning and curriculum writing of groups being lead.  These curriculums are to be completed and approved by the CAGS Clinical Supervisor prior to all group sessions and are due prior to the commencement of any new therapeutic group.
  9. Curriculums must be based upon individual client treatment goals and supported by treatment team plans.
  10. Responsible for following the planned curriculum and documenting when there are changes from the clinical planning for clients.
  11. Participate cooperatively and flexibly in daily client transportation to and from CAGS. 
  12. Report all special incidents and documents the same to CAGS Supervisor.
  13. Attendance to staffings, as necessary for youth and family.
  14. Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
OTHER DUTIES
  1. Stay current with all training requirements.
  2. Attend scheduled staff meetings, supervision, and on-going training.
  3. All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
  4. Respect the privacy of clients and hold in confidence all information obtained during the client’s treatment.  All client-related documents should be handled in accordance with Agency guidelines on confidential material.
  5. Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  6. Other duties as assigned by management.

  • Oversee end-to-end management of multiple large-scale programs (ranging from $5M to $30M in annual contract revenue value), ensuring all deliverables are met within scope, budget, and schedule.
  • Develop and implement strategic initiatives to drive program efficiency, cost reduction, and performance enhancement.
  • Foster strong relationships with government clients, contractors, and regulators acting as the primary point of contact for program status and escalation.
  • Monitor compliance with federal regulations, ensuring adherence to agency standards.
  • Prepare detailed reports and present program metrics and vision to senior leadership, driving decision-making processes.
  • Identify business growth opportunities and collaborate with business development teams to secure future contracts and renewals.

Education, Experience and Requirements

  • Bachelor's degree in Environmental Science, Geology, Engineering or a related field (required); MBA or Master’s in Environmental Science (preferred).
  • Prior experience with the Ohio Voluntary Action Program, and USEPA and State of Ohio brownfield grants.
  • 40-hour HAZWOPER Training (preferred).
  • 10+ years of managing client accounts and business development efforts.
  • Minimum 20 years’ relevant experience in consulting, industry or government agencies.
  • In-depth understanding of government contracting procedures, federal acquisition regulations (FAR), and experience with IDIQ contracts (Indefinite Delivery/Indefinite Quantity) (required).
  • Must receive a satisfactory pre-hire drug screen and background check.
  • Comfortability working outdoors and traveling up to 25% of the time interstate.
  • Must be able to meet the physical requirements.
