Home Therapies RN CAP 1

The registered professional nurse Home Therapies RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. Assesses and manages patients' response to home dialysis training and treatment therapy by following prescribed predetermined protocols and communicates patient related issues to the physician as needed. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.

All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations.

Performs all essential functions under the direction of the Supervisor with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse.

Performs ongoing, systematic collection and analysis of dialysis data for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Supervisor or physician as needed.

Assesses, collaborates and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.

Directs and provides, in collaboration with the patient, home care partner, direct and ancillary patient care staff, all aspects of the provision of safe and effective delivery of dialysis therapy to assigned patients.

Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness.

Initiates or assists with emergency response measures.

Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.

Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.

Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.

Ensures patient awareness related to transplant and treatment modality options.

Assists in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners.

Performs assessment and identifies barriers of the Home Dialysis candidate's home environment and partner / family readiness and ability to perform dialysis treatments in the home.

Trains Home Dialysis patients and / or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies through an organized and formal Home Dialysis Training Program.

Required to complete CAP requirements to advance.

Performs all other duties as assigned by Supervisor.

Assigned oversight of LPNs/LVNs, RNs, Patient Care Technicians and Home Therapy Care Team Assistants as a Team Leader or designated Nurse in charge, after meeting all the following:

Successful completion of all FKC education and training requirements for new employees.

Must have a minimum of 12 months experience as a RN.

Successful completion of 3 months experience as a RN in home peritoneal dialysis and / or hemodialysis.

Graduate of an accredited School of Nursing.

Current appropriate state licensure.

Current or successful completion of CPR BLS Certification

Must meet the practice requirements in the state in which he or she is employed.

Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse.

Minimum of 1-year experience as a Registered Nurse (preferred)

Home dialysis therapy experience (preferred).

  • Encounters - All encounters to be completed daily by physician as this affects hospital funding.
  • Consults Closure: Consults are required to be completed by the provider at the time patient has been seen.

  • Budget Development and Management: Ensure financial resources are planned and expended to fulfill needs within budgetary limits, policies, procedures, and deadlines. Monitor compliance with all Environmental commitment and/or permits and the Project-Specific Stormwater Pollution Prevention Plan. Coordinate with Environmental section to ensure compliance with any environmental requirement and ensure Field Maintenance Leadership has uploaded required information into the Comprehensive Environmental Data and Reporting System.
  • Equipment: Effectively manage equipment resources and business activities. Monitor equipment preventive maintenance program. Ensure work units have needed equipment, or use alternative means to secure assets needed to complete work. Work with Field Maintenance Leadership to coordinate submittal requests of replacement equipment and partner with respective equipment shop supervisor to prioritize needs.
  • Fiscal, Inventory, and Procurement: Effectively manage resources to support the District's Dashboard targets. Promote sound, ethical business transactions to ensure compliance with policies and procedures. Oversee purchasing, inventory, and accounts receivables. Monitor established IMS goals and manage inventory based on IMS policies and procedures. Respond to local audit findings and ensure proper resolution to irregularities.

  • Assemble warp beam sets, including checking & changing shaft bearings

  • Check new warp to make sure all beams are lined up and collars are bolted in place
  • Make sure all end caps are tightened

What to Expect After You're Approved:

StylesGo launches city by city. Once you're approved, it can take a couple of weeks before appointments start coming in while we bring on enough barbers and stylists in your area to open it to clients. Approved providers are notified the moment their area goes live, and being on the platform early means you're first in line when it does.

About the Role
The Project Manager oversees the successful execution of residential roofing and exterior restoration projects throughout the Madison area. This role is ideal for someone who enjoys being in the field, solving problems, supporting crews, and delivering an exceptional customer experience.
You'll play a critical role in managing active projects, coordinating with homeowners and crews, maintaining quality standards, and ensuring projects are completed safely, efficiently, and on schedule.

Responsibilities
  • Manage construction projects and coordinate with crews
  • Travel between job sites, monitor progress and conduct inspections
  • Act as main point of contact with homeowners
  • Help crews overcome challenges that could impact quality, safety, or scheduling
  • Document milestones, coordinate materials, and coordinate crews
  • Communicate with internal teams, and proactively solve issues before they become tangible problems.
Requirements
  • Experience coaching, mentoring or managing teams, crews or field personnel
  • Strong understanding of residential roofing and exterior construction best practices
  • Strong problem-solving, communication and organizational skills
  • Valid driver's license
  • Proficiency with smartphones, PM sofware, CRMs, and Microsoft Office Suite
  • Comfortable climbing on ladders and working outdoors
Schedule
40 hour work weeks Monday through Friday. Some Saturdays might be asked of you.
