Charge nurse may or may not have a patient assignment. - Phlebotomy, IV/VAT team, telemetry tech, RT, hospitalist, and intensivist support available 24/7. - Uses VNO (virtual nursing - 10 cameras). Virtual RNs can act as 2nd RN witness for certain meds, closely monitor patients requiring special monitoring, and assist with admission/discharge items.

Shift & Scheduling: - Weekend rotation: every other weekend. - Possibility of low census call. Can be called back up to 3a for Nights and up to 3p for Days. - Travelers will work 75% of holidays. - Schedule cycle: every 6 weeks.

Unit Details: Patient ratios are 1:4 for tele/pediatrics, 1:3 for the step-down unit, and 1:5 for med-surg. The unit uses Omnicell as its medication system. No IV team or lift team is available; pharmacy, phlebotomy for lab draws, and a unit secretary are available 24/7. Bedside chest tube procedures are performed very rarely. Hospitalist coverage is available 24/7; an intensivist consult may be requested through the hospitalist 24/7.

Shift & Scheduling: Associates can be placed on call depending on census. Full-time staff work 2 weekend shifts per pay period; part-time staff work 1 weekend shift per pay period. Some staff have a set schedule (rotation), while some staff are flex.

Floating Requirements: You may be required to float to Adventist Health locations within 30 miles.

As a Home Health LPN , you will:

  • Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
  • Implement individualized care plans in collaboration with the patient, family, and healthcare team.

About the Role

As a Home Health Physical Therapist, you'll provide one-on-one rehabilitation care in patients’ homes, helping them recover from surgery, injury, or illness while improving function, reducing pain, and supporting greater independence.

This role is ideal for a licensed Physical Therapist who values autonomy, clinical variety, and the ability to build meaningful relationships with patients over the course of their recovery. Support services include physical therapy, respiratory services, interpretation services, radiology, social services, rapid response teams, pharmacy, transportation, and a unit secretary.

Shift & Scheduling: Every other weekend.

Floating Requirements: Float assignments include Neuro ICU, Surgical ICUs (including SICU), Medical ICUs (MICU x2), NCCU, CVICU, and Cardiology/Diagnostics, based on staffing and patient acuity. The Critical Care Float Pool covers ICUs throughout the Ohio State system and may float throughout the organization if there is no need in Critical Care. Nurses are required to float within units of equivalent or lower acuity.

14 days maximum time off. Start assignment restriction: 7 days
