REPORTS TO: Direct Supervisor/Director of Advanced Practice Professional of the Heart and Vascular Institute

FLSA STATUS: Exempt

POSITION SUMMARY: Coordinates and manages the care of the CT surgery patient across the continuum of care. Works in collaboration with the CT Surgery providers to ensure comprehensive acute/ long-term care for patients managed by the service. Helps develop the team to meet quality and performance measures for CT Surgery. The Advanced Practice Professional (APP) coordinates involvement of medical/surgical specialty teams necessary to develop a comprehensive plan for the patient, and reviews the plan of care with the other members of the clinical team, participates in directing that care. The APP will order/perform diagnostic/therapeutic procedures, interpretation of such studies, will be involved with all aspects of the peri-operative services and coordinates discharge/follow up planning. The APP will work with the team to ensure adherence to protocol requirements and regulatory guidelines. They will ensure protocol compliance and close monitoring of order sets. The APP participates in the education of residents and medical/APP/nursing students.

Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable. Such service may include but are not limited to.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  • Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing
  • National Certification by one of the recognized APRN Certification Centers
  • Licensure, or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia
  • BLS certification required. Additional life support certification at the discretion of the department
  • Department Specific: APRNs requesting to be privileged as First Assists in peri-operative services will be required to complete an ANOR approved CNOR Certificate program and obtain a passing grade on CNOR certification exam
  • Advanced Cardiac Life Support (ACLS) at the discretion of the department

EXPERIENCE:

  • Eligible new graduates will be considered

PREFERRED QUALIFICATIONS:

  • CT Surgery, Acute care or cardiology experience preferred

MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  • Bachelor’s OR Master’s Degree as a Medical/Surgical (as applicable) Physician Assistant required
  • Graduate of an ARC-PA approved Physician Assistant (PA) training program required
  • Certification by the National Commission for Certification of Physician Assistants required
  • Must be licensed or eligible for licensure by the WV Board of Medicine
  • BLS certification required. Additional life support certification at the discretion of the department
  • Advanced Cardiac Life Support (ACLS) at the discretion of the department

CORE DUTIES AND RESPONSIBILITIES

  • Manages comprehensive and individualized care for cardiothoracic surgery patients and their families in the inpatient phase of hospitalization and immediate post discharge in the ambulatory setting
  • Completes daily rounds: consults and/or pre-procedure evaluations during weekdays and as applicable, weekends/holidays as determined by specific clinical site and specialty
  • Completes integrated assessments, documents efficient and precise clinical evaluations/plan in the electronic medical record (EMR)
  • With the collaboration of faculty or by following established clinical guidelines and protocols, orders additional labs and/or diagnostic studies or consults as deemed appropriate
  • Performs, interprets and/or communicates results of diagnostic tests following established clinical guidelines and scope of practice in the inpatient and outpatient setting
  • Communicates to patient•s comprehensive care team results of labs / diagnostic studies following established clinical guidelines and scope of practice, regarding abnormal findings in the evaluation of the patient
  • Develops a comprehensive individualized plan of care for CT Surgery patients, which includes teaching, discharge planning and necessary referral to other members of the health care team as necessary
  • Identifies unstable or complex patient care situations and facilitates appropriate intervention with appropriate resources, in the clinic and/or hospital setting
  • Recognizes emergency situations and initiates effective emergency care
  • Facilitates patient discharge by coordinating appropriate services and tests prior to discharge necessary for outpatient follow-up appointments in the clinic (as applicable)
  • Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification
  • Possess and/or acquire the appropriate surgical skill set to be able to first assist in all CT surgery operations performed by the service (as applicable)
  • Possess and/or acquire the appropriate surgical skill set to be able to perform open and/or endoscopic harvesting of appropriate conduit during the CT surgery operation (as applicable)
  • Assists in the collection of outcome measures (as applicable)
  • Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual needs of patients/families
  • Demonstrates the ability to utilize equipment in a safe manner
  • Bases decisions made/actions taken on the relevant scientific principles, established standards of care and/or research findings
  • Utilizes teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, students, patients, families and the community
  • Promotes an environment that facilitates learning by responding to appropriate patient/family preferences, priorities and readiness to learn
  • Updates own knowledge base throughout the year
  • Completes all identified clinical competencies
  • Serves as a resource person for staff caring for the CT surgery patient
  • Participates and initiates inter-disciplinary approaches to patient care, program development and education. Serves as a professional role model

POSITION SUMMARY

The Advanced Practice Professional (APP) will continue with the core duties and responsibilities outlined in the job description as an APP within the Heart and Vascular Institute Cardiothoracic Surgery service line. Coordinates the clinical aspects related to the Heart and Vascular Institute including education, advancement of skills and maintenance of the APP scope of practice.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER

Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable. Such a service may include but is not limited to:

  • Graduate from a CCNE accredited nursing program with a master's degree in nursing.
  • National Certification by one of the recognized APRN Certification Centers.
  • Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
  • American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.

EXPERIENCE

  • Must have at least three years’ experience in Cardiothoracic Surgery.

PREFERRED QUALIFICATIONS

  • ACNPs or FNPs.

MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT

  • Bachelor's degree or master's degree as a Physician Assistant is required.
  • Graduate from an ARC-PA approved Physician Assistant (PA) training program required.
  • Certification by the National Commission for Certification of Physician Assistants is required.
  • Must be licensed or eligible for licensure by the WV Board of Medicine or WV Board of Osteopathic Medicine.
  • American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.
  • Master’s Degree as Physician Assistant preferred.

CORE DUTIES AND RESPONSIBILITIES

  • Manages inpatient APP personnel to ensure the availability of appropriately credentialed, clinically competent, and well-developed cohesive work force to meet patient care needs. Navigates credentialing/privileging and facility directives at regional sites to assure adherence to local policies.
  • Functions as a staff role model by promoting cooperative problem solving, positive progressive discipline, individual and group goal orientation, appropriate delegation of division duties/activities and the building of the esprit de corps through individual coaching and counseling, regular attendance at meetings, assisting with annual performance appraisals, and actively participates in the activities of the division.
  • Assists in the development and implementation of ongoing educational programs for professional and support staff that include new employee orientation, in-service continuing education, procedural training, and new equipment and/or systems training which enable the staff to perform based on current policy/procedures and state-of-the-art practices.
  • Manages the inpatient personnel to ensure adequate APP coverage in the inpatient settings to meet the patient's needs.
  • Maintains effective communication with Medical Staff, patients, staff, and other departments as necessary to assure identification of problems and provide problem resolution in support of the hospital’s mission of quality care delivery.
  • Communicates effectively with physicians, nurses and other personnel in problem identification and resolution in a timely manner.
  • Works in conjunction with the staff and department leadership of HVI to disseminate necessary information to others by written and/or verbal means.
  • Works in conjunction with the leadership of staff and physicians in identifying areas of opportunity for improved efficiency and improvement in quality care delivery utilizing the Performance Improvement process.
  • Communicates employee concerns to the appropriate personnel, as applicable.
  • Plans, organizes, implements, and evaluates the delivery of care in accordance with Standards of Practice and Standards of Care.
  • Collaborates with medical staff of assigned facilities to ensure that APP care is consistent with local and regional medical staff procedures.
  • Facilitates the professional development of personnel. Oversees and participates in the orientation, training, and continuing education of the staff (departmental and interdepartmental) and other health related personnel.
  • Directs and monitors the instruction, supervision, and evaluation of students and new employees.
  • Assures continuous personal and professional growth through attendance at workshops, seminars, giving in-services, and monitoring staff orientation plans as applicable.
  • Identifies current and anticipates future departmental educational needs. Collaborates with department leadership and staff in the development of departmental education plans, including evaluation of the process and outcome, and facilitation of staff participation.
  • Improves and maintains personal management skills by participating in self-development activities such as seminars, workshops, classes, and literature review.
  • Participates in research and quality initiatives.
  • Other duties may be assigned by the department leadership or Chair of HVI.

PHYSICAL REQUIREMENTS

  • Heavy/Hard Work: Work requires strength and/or stamina, lifting, moving, stooping, reaching, positioning of patients, standing, walking, and carrying of patients, materials and equipment weighing 40+ pounds.
