We are seeking a Registered Nurse Home Care Coordinator CarePatners for our PACE (Program of All Inclusive Care for the Elderly)  team to ensure that we continue to provide all patients with high quality, efficient care. Job Summary and Qualifications
  • Role Summary:

Under the supervision of the Operations Director, plans, organizes and implements in home services for PACE participants and families. Responsibilities include but are not limited to: Coordination of the day-to-day operations of the In Home Services for your team of participants. Act as the liaison between the contracted home health agency and the PACE program coordinating homecare Nurses, Home Health Aides, Homecare Services Non-Skilled Aides and the Homecare Schedulers. Utilization of nursing skills to assess participants in their home environment and coordinate plans of care with appropriate resources and provide treatments and health education for participants as appropriate. Participation on interdisciplinary team to ensure compliance with competency requirements, maintaining medical records and thorough documentation. Planning and using telehealth, assistive technology, and community resources, to create and implement health and wellness in homes.

What qualifications you will need:
  • Skills, Knowledge, Abilities:

    Required Education:

    Bachelor of Science in Nursing. Nurses with an Associate Degree or Diploma in nursing must sign agreement to obtain BSN within 6 years of the hire date in to position. Nurses who are nearing the end of their professional career may be exempted from this requirement with CNO System Council approval.

    Required License:

    · Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. Compact license may apply, licensee should confirm with NCBON; BCLS

    · Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$50K Property Damage coverage) and have reliable transportation

    Required Experience:

    · Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting

    · Minimum of 1 year working with a frail or elderly population.

    · Willingness to work in home environments of patients who may have values or standards different from your own

    Preferred Experience:

    · Computer literacy with EMR and familiarity with telehealth and providing consultations over a video platform

    · Nursing in a community home based setting preferred: 3 years

    · ACTT RN or Mental Health experience: 2 years

    Job Summary and Qualifications

    As a Registered Nurse (RN) in the Cardiac Cath Lab, you will be responsible for nursing care and related assistance for adult and geriatric patients with a broad range of illnesses. You will handle assessment of patients, medication administration, direct and indirect patient care, circulating cases, scrubbing procedures and procedure documentation and monitoring.

    * You will complete and document a patient assessment upon admission

    * You will reassess and document changes in patient condition

    * You will administer medications and monitor and document response

    * You will monitor the patient’s overall status pre, during, and post procedure

    * You will communicate any patient changes to the physician

    * You will manage patients with a potentially life-threatening dysrhythmia or hemodynamic change via physician order by using medications and emergency equipment

    * You will explain procedures to patients and assess their understanding


    Job Requirements


    Experience

    * 1 year of clinical nursing experience as a Registered Nurse (RN)

    * 1 year of Cath Lab experience in a cardiovascular setting – preferred


    Education

    * Associate’s degree in Nursing (ADN)


    Licenses and Certifications

    * Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License, Texas license must be obtained within 90 days of hire

    Job Summary and Qualifications

    As a Neonatal Intensive Care Unit (NICU) Registered Nurse (RN), you will be responsible for performing patient assessments, developing a plan of care, and implementing and evaluating care on infants. As a member of the interdisciplinary care team, you will work collaboratively with the other members to assure the safe and accurate delivery of care.

    * You will attend deliveries and performs neonatal stabilization and admission assessments of newborns

    * You will create a plan of care, including discharge plans, revise the plans based on the patients’ response to treatment and regularly evaluate plans for effectiveness

    * You will conduct on-going assessments of infants and communicate any changes to Nurse Practitioner or Neonatologist

    * You will administer medications and complete treatments according to hospital policy

    * You will assist transport teams in stabilization of infants and provide necessary support and documentation to facilitate the transfer

    * You will provide support and education materials to families on homecare, treatments, adoption and support groups

    * You will assist in high-risk deliveries and assist Nurse Practitioner with resuscitation

    * You will delegate work appropriately to non-licensed staff in accordance with standards of practice


    Requirements


    Experience

    Become the Director of Therapy Operations you've always aspired to be

    • Develop, revise, implement, and communicate policies, processes, and procedures, holding staff accountable for their respective roles and responsibilities.
    • Collaborate with the marketing team to educate staff and the community about awareness, prevention, and treatment options. Develop and support clinical affiliations and relationships with educational institutions.

    Be the Chief Nursing Officer you've always aspired to be

    • Collaborate with all medical, clinical, and therapeutic disciplines to foster a patient-focused, team-oriented culture that delivers superior service and outstanding outcomes.

    Be the Pharmacy Director You've Always Strived to Be


    • Convey the mission of the hospital and organization to pharmacy staff, holding them accountable for performance, while motivating them to enhance and develop their capabilities.
    • Oversee the pharmacy department's operations, including scheduling, continuous assessment, and the implementation of process improvements to optimize department performance.
    • Provide direct patient and pharmaceutical care.
    • Demonstrate and maintain competence in clinical health system pharmacy practices, including antimicrobial stewardship, non-sterile and sterile drug preparation, controlled substances, hospital pharmacy accreditation, anticoagulation drug therapy management, and medication safety.
    • Educate staff on pharmaceutical care and medication management, ensuring the pharmacy consistently complies with local, state, and national regulations.
    • Stay current with new medications, indications, and evidence-based medicine. Collaborate with the Controller to prepare and manage departmental budgets for staffing, drugs, and supplies.
    • Actively serve and lead various hospital committees, including Quality Assurance, Formulary, Pharmacy and Therapeutics, MEC, and Safety, as needed.

    Qualifications

    • Licensed in good standing by the State Pharmacy Board for the state where the hospital is located.

    Harbor House of Central Florida is Orange County's state-certified Domestic Violence service provider. We are dedicated to supporting survivors of domestic violence through comprehensive services, including a 24-hour crisis hotline, emergency shelter, counseling, legal advocacy, and community education.

    Position Overview: The Development Assistant provides administrative and operational support to the Development team to help achieve fundraising and donor engagement goals. This role is responsible for maintaining donor databases, preparing materials for fundraising campaigns and events, assisting with communications and donor stewardship, and ensuring the smooth daily operations of the department.

    Key Responsibilities:

    • Manage and maintain the Development Department calendar.
    • Provide general office support to the Development Department.
    • Assist with Donor prospecting- In kind and monetary.
    • Maintain and manage donation data entry with clean entries for donors, volunteers and event attendees, along with Segment contacts for appeals, stewardship and re-engagements.
    • Acknowledgement of Donors through various methods of communication and contact including newsletters.
    • Assist with Tabling and Harbor House general speaking events about Harbor House.
    • Assist with Donation pick-up and delivery.
    • Assists with all silent auctions and sponsor benefits.
    • Assists with Gifts of Hope Harbor House event.
    • Assists with all Harbor House events for the needs of printing, mailing, event attendees and logistics.
    • Assists with all Harbor House mailings.
    • Assists with volunteer communication, recruitment, training and reporting.
    • Assist with social media: drafting media posts, CLASSY emails, monitoring and updates.
    • Provides assistance post events through various tasks regarding thank you, photos, evaluations, and reports.
    • Provides assistance with community requests for educational materials.

    Qualifications:

    • Education: Associate's Degree in, Business Administration, Public Relations, or Communications preferred.
    • Experience: 1-3 years' experience in administrative or fundraising (non-profit experience a plus).
    • Discretion: Professional experience may be substituted for formal education at the discretion of the Chief Executive Officer.
    • Licensing: Valid Florida Driver's License with a clean record for the past 5 years. Must be 21 years or older to meet agency insurance requirements.
    • Skills: Proficiency in Microsoft Office applications (Word, Excel, PowerPoint). Ability to learn organization-specific software, including databases and content management systems. Strong professional writing and communication skills. Ability to manage multiple priorities in a fast-paced environment with attention to detail.
    • Training Requirements: Completion of CORE Competency training within 90 days of hire. Additional mandatory training is required on an annual basis per Agency Policies & Procedures.
