What You'll Do Targeted Case Managers help individuals with behavioral health needs access, coordinate, and maintain the services and resources necessary to function successfully in the community.Responsibilities include: Assess client and family needs and identify appropriate community resources and supports. Link and coordinate clients with medical, behavioral health, educational, social-service, housing, transportation, benefits, and other community resources based on individual needs. Provide Targeted Case Management services in accordance with client service plans, Medicaid requirements, and agency policies. Maintain regular communication and coordinate care with clients, caregivers, treatment providers, schools, medical professionals, and other community agencies as appropriate. Advocate for clients and assist them in navigating complex service systems. Complete required documentation accurately and within established deadlines. Provide culturally responsive services and accommodate clients' language and communication needs. Maintain knowledge of community resources within the geographic area served. Participate in training, supervision, quality assurance, and other professional-development activities. Minimum Qualifications Candidates must have: A bachelor's degree from an accredited college or university. Relevant work experience in behavioral health, mental health, social services, case management, child welfare, human services, or a related field. Current CBHCM, CBHCM-P, or CBHCMS certification is preferred but not required for otherwise-qualified candidates. Candidates who are not yet certified must be willing to complete required training and pursue applicable Florida Certification Board certification. Ability to communicate effectively in English, verbally and in writing. Strong organizational, documentation, communication, and time-management skills. Ability to work independently while remaining an active member of a collaborative team. Ability to work effectively with individuals and families from diverse backgrounds. Comfort using electronic health records and other basic computer applications. Bilingual candidates are strongly preferred , particularly those fluent in English/Spanish or English/Haitian Creole . Community-Based Position Targeted Case Management is a community-based service .Candidates must be willing and able to travel throughout their assigned service area to meet with clients and community providers.Applicants must have a valid driver's license, reliable transportation, and an acceptable driving record .We are currently especially interested in candidates able to serve: Cape Coral Fort Myers Lehigh Acres Surrounding Lee County communities Training & Certification Support One of the advantages of joining Stellar is that we provide Behavioral Health Case Management education directly.As a Florida Certification Board Certified Education Provider , Stellar offers the required 50-hour Behavioral Health Case Management course and can provide training to eligible candidates who need to complete this requirement.Applicants who already hold CBHCM, CBHCM-P, or CBHCMS credentials are strongly encouraged to apply. Additional Requirements Candidates must be authorized to work in the United States and successfully complete required background screening and onboarding requirements.We are looking for self-starters who are comfortable developing relationships in the community, learning local resources, and participating in outreach and referral development.Stellar may provide referrals and caseload opportunities; however, TCMs are also expected to actively participate in the continued development of their caseload and community referral relationships.
