Saint Alphonsus Medical Group (SAMG) is a physician led 650+ provider multispecialty group with an excellent opportunity for a fellowship-trained vascular surgeon to join a high-performing practice that has grown 30% in the past 2 years and achieves excellent patient satisfaction scores. Our outstanding, 3-Surgeon, 2-APP vascular surgery program is looking to add a vascular surgeon possessing knowledge and experience with vascular surgical techniques relating to advanced and complex open as well as endovascular operative treatment of all manner of vascular disorders. The ideal candidate will commit to a multi-disciplinary team approach to patient care, desiring to contribute to the further expansion of a regional vascular referral center in support of a growing multi-specialty medical group. Vascular surgeons in the center also support a non-invasive vascular lab by providing oversight, supervision, and interpretation of vascular studies. Although not a requirement, there is a preference for the Registered Physician in Vascular Interpretation (RPVI) credential. Boise-based vascular surgeons participate with hospital administration in co-managing the vascular surgery service line. SAMG, with primary and specialty care providers at 72 clinic locations, is also a member of the Saint Alphonsus Health Alliance, a network of over 1,800 employed and independent providers.

Support: Vascular surgery is supported by a 24/7 surgical call team specifically dedicated to the Vascular Surgery Service. Saint Alphonsus Regional Medical Center has invested in Vascular Surgery, installing a $4.2M Hybrid O.R. with Philips FlexMove technology. This room integrates a surgical operating room with an advanced x-ray imaging system enabling our surgeons to perform the most advanced endovascular procedures. Saint Alphonsus Regional Medical Center installed a second Hybrid O.R. which is a beta site for Phillips and Macquet utilizing Philips Azurion technology, and allowing for a 50-75% reduction of radiation exposure to both staff and patient during vascular procedures.

Responsibilities:
The Virtual Primary Care Family Medicine Advanced Practice Provider (Nurse Practitioner/Physician Assistant) provides comprehensive primary care to new and established patients for Jefferson Health via telehealth platform. This service will expand Jefferson Health primary care services to new and existing markets. S/he collaborates with, and is an integral member of, a multidisciplinary health care team. Patients may utilize Virtual Primary Care for continued primary care services or as a transition point when waiting for a new or established appointment in their preferred Jefferson primary care location ( Supplemental Access ). The Virtual Primary Care APP will work in conjunction with the population health team to address care gaps and provide equitable, value-based care.
Overview:
Assessment/Diagnosis and Management of Patients in Primary Care Setting
Provides patient-centered, preventive health, including well visits, screenings, and immunizations recommendations; as well as acute and complex care visits requiring diagnosis, treatment/management
Collaborates with interdisciplinary health care team.
Possesses awareness and provides assessment of patient risk factors, including physical, socioeconomic, and psychosocial risk factors.
Obtains and documents thorough patient history.
Performs and documents physical exam within the limits of telehealth.
Orders, performs, and interprets appropriate diagnostic testing, and responds to results requiring further evaluation or investigation.
Formulates differential diagnoses by priority.
Engages/Collaborates with collaborative physicians when needed (i.e. diagnosis in question, patient not responding to treatment, etc.).
Coordinates with and refers to specialists and/or a higher level of care as needed.
Complete professional billing in timely and accurate manner, in compliance with regulations.
Formulates Plan of Care
Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address preventive care, acute care, and chronic health care needs.
Utilizes evidenced-based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum.
Implements and modifies plan of care based on patient response, diagnostic testing, and specialist recommendations.

Documentation
Telehealth visits are documented in EPIC system using SOAP format.
Provides timely documentation in Epic; meets compliance standards; and captures patient history, presenting problem, physical exam within limits of virtual care, diagnosis(es) and treatment/plan of care.
Signs/closes visit notes within 24 hours of the patient encounter.
