Tallwood Men’s Health

Hartford HealthCare is continuing its dedication to personalized and coordinated care by introducing Tallwood Men’s Health, a multidisciplinary care model new to Connecticut. Men are an underserved healthcare population.  Research has shown that men utilize healthcare less, have a higher incidence of nearly all chronic diseases, and die at a younger age than women.  These conditions can be addressed by providing multidisciplinary care for men in a comfortable setting with providers focused on whole men’s health care. Multiple urologists will be practicing in a brand new location, including specialists in general urology, andrology (sexual health and fertility), stones, and urologic cancers.  Other specialists in cardiology, endocrinology, bariatric medicine, bariatric surgery, and mental health will be available to provide true multidisciplinary care.

Advanced Practitioner Role Scope

The Nurse Practitioner or Physician Assistant will work in an ambulatory setting and will be responsible for evaluation of urology patients. Advanced practitioners expressing an additional interest in cardiology, endocrinology, or bariatric medicine may also gain additional experience within these specialties. APP’s initiate patient care meetings with the health care team to revise the plan of care as the patient’s condition warrants. Participate in total quality management to positively affect patient and system outcomes to improve care. Potential coverage of nearby practice locations. Monday through Friday schedule with no call!
