Qualifications

Job Description

Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. JOB SUMMARY

The Hartford HealthCare Heart & Vascular Institute is seeking a Physician Assistant or Nurse Practitioner (APRN) leader in Cardiology at Backus Hospital as well as supporting the outpatient Hartford Healthcare Medical Group office setting to coordinate an advanced level of comprehensive health care for cardiology patients. This role specifically oversees the work of the APP team in cardiology supporting the Norwich provider group of general cardiologists and non-invasive procedural cardiologists and takes a leadership role in overall program development and quality.

RESPONSIBILITIES

  • Clinical services as cardiac Physician Assistant or Nurse Practitioner (APRN)
  • Provides overall clinical supervision, including work assignments, schedules, training and evaluation in accordance with specific, respective departmental policies and procedures in collaboration with the physician leadership and practice manager.

As part of our 24/7 APP-driven service, you will practice with a high level of autonomy while being fully supported by a collaborative team of expert neurosurgeons and experienced APPs. Our team provides exceptional care across all areas of neurosurgery, from minimally invasive and deformity spine surgery to advanced cranial, neurovascular, and neurotrauma cases.

In addition to providing comprehensive neurosurgical patient care, the Lead Advanced Practice Provider serves as a clinical leader and resource for the Neurosurgery APP team. This role supports clinical excellence through mentorship, onboarding, education, quality improvement, and collaboration with physician and operational leadership while maintaining an active clinical practice.

The Lead APP is a frontline clinical leadership role that supports practice excellence, professional development, quality initiatives, and team effectiveness, but does not have direct personnel management responsibilities.


Position Highlights

Clinical Responsibilities (70%)

  • Manage neurosurgical spine and cranial patients in both the ICU and inpatient floor settings.
  • Perform comprehensive clinical responsibilities, including intake H&Ps, progress notes, consultations, postoperative management, and discharge planning, as well as providing neurosurgical evaluations throughout the hospital, including the Emergency Department.
  • First assist in the operating room for a wide range of emergent and elective cranial and spine procedures.
  • Participate in interventional radiology coverage for neuroendovascular cases.
  • Support the full scope of emergency and elective neurosurgical care across the service line.
  • Participate in a collaborative, 24/7 APP-covered neurosurgical service supporting inpatient, procedural, emergency, and perioperative patient care.

Lead APP Responsibilities (30%)

  • Serve as a clinical resource, mentor, and role model for Neurosurgery APP team members.
  • Support onboarding, clinical education, and professional development of APP staff.

Nurse Practitioner and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Advanced Practitioners perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Advanced Practitioners develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Advanced Practitioners provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Summary

  • Provides quality, holistic and cost-effective advanced practice care for patients, families, and others in the women’s health department to achieve and/or maintain optimal health status, manage disease and reduce risk of illness. 
  • The primary responsibility of this position is surgical first assisting for gynecologic procedures, including minimally invasive, robotic, vaginal, and open surgeries. Additional duties include patient management and consultative services on the Women’s Health surgical units within Hartford Hospital; including pre- and post-operative care, inpatient and emergency department consultations, and coordination of care with multidisciplinary teams and other departments, including the Emergency Department
  • This position entails working closely with the UCONN Obstetrics and Gynecology residents as part of the women’s health inpatient services.
  • This is a 40-hour position, primarily weekday shift coverage.
  • Schedule includes 4–10-hour shifts Monday through Friday, 6a-4p or 8a – 6p. 
  • The position also requires some flexibility several times a year to help cover evening hours for resident events such educational retreats, in-service exams and graduation.

Qualifications

  • Current Connecticut PA/APRN license

Job Description 

This individual is responsible for supporting the multidisciplinary care and coordination of patients with PH across the inpatient and outpatient settings while helping ensure ongoing compliance with PHA accreditation standards and best practices.

The PH Nurse Navigator functions as a central liaison between patients, physicians, advanced practice providers, specialty pharmacies, nursing staff, respiratory therapists, pharmaceutical representatives, hospital departments, and external referring providers. The nurse plays a key role in facilitating seamless transitions of care, optimizing patient access to PH therapies, supporting clinical operations, maintaining accreditation requirements, and enhancing the overall quality of care delivered to PH patients.

  • Navigates patients through the continuum to eliminate barriers to health care and enhancing patient and physician satisfaction.

  • Function as collaborating member of the health care team, but also must be able to act independently to assess patient and physician needs to ensure appropriate hand-offs so that patients receive services promptly and efficiently.

  • Applies the nursing process as appropriate within the context of the organization’s framework for professional practice and in accordance with guidelines established by the team.

  • Participate as needed in community education outreach programs/workshops/seminars/support services.


Qualifications


  • Five years’ experience as a nurse OR One year experience within the specialty (Cardio/Pulmonary Disease)

  • Two years supervisory/management and patient/staff education experience preferred
