Job Summary

Under the general direction of nursing leadership and with provider collaboration and guidance, the Registered Nurse utilizes evidence-based practices in accordance with the State Nurse Practice Act and policies and procedures of Hartford Healthcare Hospital Outpatient Departments (HODs) and Medical Group, in a variety of clinical settings. This position facilitates care for a varied population of patients, which includes nursing standards of practice to meet the physical, emotional, spiritual, cultural, and educational needs of the patient and family.

  • High volume practice with high demand of exposure to nursing treatments including clinicians' patients prep support.
  • Provides nursing care nursing care within a non-acute setting in collaboration with the inter-disciplinary team and provider according to the patients physical, emotional, spiritual, cultural, and educational needs of the patient and family.
  • Coordinates and provides holistic patient centered care.
  • Utilizes evidence-based practice in accordance with the department and/or medical group policies and procedures to ensure safe quality care and customer service.
  • Incorporates the art, advocacy, science and ethics in nursing effectively in their daily practice.
  • Demonstrates and actively supports research utilization through inquiring into evidence-based practices.
  • Evaluates and reviews the plan of care working with the interdisciplinary team to meet the diverse needs of the patient and family.
  • Identifies and reliably documents problems and expected outcomes based on a plan of care for a varied population of patients.
  • This role requires coverage at regional offices bringing variety to your workday and the chance to collaborate across multiple teams and communities.

Qualifications


Education

•        Graduate of an accredited School of Nursing.

•        Associate’s degree in Nursing; BSN, preferred

•        Successful completion of nurse residency, if applicable.

Experience

•        One year of experience with a minimum of 1000 hours of clinical experience as a licensed registered nurse.

Licensure, Certification, Registration

Job Description

Position Details:

Job Description

Responsible for coordinating the individualized care of patients throughout the continuum of care services for patients with digestive health issues in order to increase and assure timely access to healthcare services.

·         Serves as a member of the multidisciplinary care team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.

·         Navigates patients through the continuum to eliminate barriers to health care and enhancing patient and physician satisfaction.

·         Functions as a collaborating member of the health care team, but also must be able to act independently to assess patient and physician needs to ensure appropriate handoffs so that patients receive services promptly and efficiently.

·         Applies the nursing process as appropriate within the context of the organization’s framework for professional practice and in accordance with guidelines established by the team.

·         Participates as need in community education outreach programs/ workshops/ seminars/ support services

·         Effectively and efficiently coordinates, implements and monitors the patient’s process of care, to communicate openly and empathetically with patients and their family members

·         Works independently as well as collaboratively with the navigation team and multidisciplinary care team in all aspects of defining and implementing navigation program.

·         Communicates effectively with diverse audiences including professional staff, patients, community, media and other service agencies.

·         Perform other related duties as required


Qualifications


·         Active CT RN License

·         Minimum of 5 years clinical nursing experience

·         BS in Nursing preferred

We are seeking a Registered Dietitian to join the Monte Nido Rosewood  team. The Registered Dietitian is a critical member of the leadership and clinical teams and collaborates routinely with all disciplines to achieve optimal client care.

Responsibilities Include::
  • Full oversight of meals prepared for clients on campus, and proper documentation.

  • Insures all nutritional services meet established standards and regulations.

  • Conduct nutrition assessments of clients, and monitor nutrition intake.

  • Participate in the treatment team’s development of the Master Treatment Plan.

Qualifications::
  • Bachelor’s degree and a credential as a Registered Dietitian OR

  • Master's degree  and the ability to get licensed within a specified timeframe 

  • At least one year working (or internship) in healthcare; psychiatric setting preferred.

  • Must have (or obtain) CPR/BLS before starting work.

  • Strong computer skills.

Summary

The Registered Nurse (RN) will be joining our newest Cardiac Evaluation & Testing Center where we provide non-invasive cardiology, interventional cardiology, cardio-oncology and electrophysiology services in the Norwalk area and across the region. Our Registered Nurses (RN) work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse workspace. As a Registered Nurse (RN), you’ll be joining our one-of-a-kind center, the first in the state to offer a streamlined approach to urgent cardiac care outside of the hospital emergency room.
