Life Care Centers of America facilities operate as Skilled Nursing Facilities (SNFs) that provide: 

  • Long term care for residents who require ongoing skilled support and compassionate daily assistance

Position Summary

The PT - Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

  • Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)
  • Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
  • One (1) year experience preferred; new grads welcome to apply
  • CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

Specific Job Requirements

The RN Infection Preventionist evaluates the quality of resident care and outcomes as they relate to Healthcare Acquired Infections (HAI) and Community Acquired Infections (CAI) in accordance with all applicable laws, regulations, and Life Care standards. Collects, prepares, and analyzes HAI data. Presents infection data and makes recommendations for actions. Monitors associate compliance with infection control standards through use of barriers and infection prevention measures. Prepares and presents education for the staff, residents, and families. Education, Experience, and Licensure Requirements

  • Perform preventative maintenance within DOT standards.

  • Receive road calls, assign third party vendor, and work with business partners on plan of action.

  • Maintain a clean and safe work environment in compliance with workplace safety rules/regulations and government guidelines.

  • Utilize fleet maintenance software system tablet to fully document equipment repair and parts used.

  • Backup to other maintenance shop personnel.

  • Willingness and capacity to assume increased responsibility and certificates.

  • Mentor other maintenance shop personnel.

  • Other duties as assigned by manager.

QUALIFICATIONS

  • Must have 3 years of verifiable hands-on experience as a Fleet Reefer/Refrigeration technician. TK/Carrier Refrigeration Certifications.

Manager, Transportation will ensure a Transportation department culture focused on people, safety, service, and cost through meaningful engagements with drivers, best in-class service to customers, and safe and profitable operations. ESSENTIAL DUTIES AND RESPONSIBILITIES 
• Lead with a safety mindset that ensures that every US Foods associate, our customers, and the general public with whom our drivers interact remain injury free, always. Champion safety procedures, processes and training to create and maintain a safety-first culture in the Transportation department.
• Oversee and develop drivers and Transportation associates through one-on-one coaching, training on policy and procedures, assisting in problem resolution when required, and leading by example. Ensure all drivers and other Transportation department associates are appropriately trained in job duties as well as safety and legal responsibilities.
• Oversee and develop drivers and Transportation associates by discussing performance with Sr. Transportation Managers and creating plans to address gaps.
• Create an atmosphere in which upward communication is encouraged, and motivate associates to achieve department goals and objectives.
• Direct and supervise the work and productivity of drivers and clerical associates within the Transportation department. Drive associate engagement and retention, and create a culture that embodies US Foods values.
• Build high-performing teams by participating in interviewing and hiring Transportation personnel, in conjunction with Sr. Transportation Managers. Review performance, coach on service and safety targets, and deliver disciplinary action.
• Monitor production goals and maintain a zero-error attitude to ensure accuracy and customer satisfaction. Anticipate, analyze and troubleshoot problems with deliveries and devise cost-effective and legal solutions; act to implement same. Ensure team understands and adheres to DOT regulations.
• Identify and stop waste, and improve processes to complete work more safely and efficiently.
• Conduct routine Safety and Driver Meetings.
Oversee maintenance of driver qualification files.
• Investigate all accidents, maintain an accident file, track causes and take appropriate preventative action.

• Effectively schedule working shifts by leveraging appropriate planning tools (i.e., Microsoft Excel)
• Other duties assigned by manager.

• Direct: Union and/or non-union Drivers

RELATIONSHIPS
• Internal: Transportation leaders and coworkers; Drivers; Warehouse leaders; Internal customers across departments (e.g., Sales)

WORK ENVIRONMENT
• May spend significant amount of time on the road. Must spend portion of working time in a truck with drivers, exposed to noise and vibration levels which may be higher than those typically experienced in passenger cars. Will spend time in an office working on a computer.

MINIMUM QUALIFICATIONS
Related Experience/Requirements:
• Minimum of three years of experience in transportation/delivery or warehouse distribution required.
• Minimum of one year of experience overseeing a workforce required.
• Experience as a driver a plus.

Knowledge/Skills/Abilities: 
• Broad knowledge of transportation/delivery operations, methods and procedures.
• Strong leadership, communication and people development skills.
• Strong understanding of DOT requirements. Familiarity with inventory control, OSHA, and other regulatory requirements.

BASIC PURPOSE: To provide a safe and reliable fleet by performing preventative maintenance and proper repairs on all fleet equipment.

ESSENTIAL DUTIES AND RESPONSIBILITIES  

  • Independently Troubleshoot, diagnose, rebuild, replace and complete all required repairs on diesel equipment (i.e., tractors, trailers, bobtails, lift gates, etc.).

  • Perform preventative maintenance within DOT standards.

  • Receive road calls, assign third party vendor, and work with business partners on plan of action.

  • Maintain a clean and safe work environment in compliance with workplace safety rules/regulations and government guidelines.

  • Utilize fleet maintenance software system tablet to fully document equipment repair and parts used.

  • Backup to other maintenance shop personnel.

  • Willingness and capacity to assume increased responsibility and certificates.

  • Other duties as assigned by manager.

QUALIFICATIONS

  • Must have 5 years of verifiable hands-on experience as a heavy-duty commercial diesel mechanic.

  • Diagnostic and troubleshooting competency relating to equipment repairs.
