Position Summary

We have an exciting opportunity to join our team as a Faculty Group Practice Physical Therapist. In this role, the successful candidate is responsible for evaluating physical therapy needs and implementing comprehensive, medically prescribed physical therapy treatment to assigned patients in a supervised program of rehabilitation.

  • Demonstrates continued proficiency in skills required to treat assigned patient populations.

Select appropriate technical aids/assistive devices and correctly refer patients to other clinical services to achieve stated program/team treatment goals. Assesses the educational needs of patients, and families, and implements a variety of methods for patient/family education and training during the rehabilitation process.
  • Completes all required documentation for patient care and reimbursement. Completes and plans all required documentation in the medical chart as required by Department guidelines and JACHO/CARF requirements. Clinical Documentation is thorough and complete using appropriate medical and clinical terminology. Accurately records all treatment given including completion of the daily treatment record.
  • Participates in Clinical Education of Fieldwork students and other professional educational endeavors. Teaches and supervises assigned students in the appropriate techniques, skills, and procedures. Demonstrates a genuine interest in assisting students in enhancing their knowledge and skill levels. Encourages positive learning experiences for Career Opportunity Students and other Non-Fieldwork students.
  • Responsible for a certain level of patient care productivity as indicated by individual Unit Procedures. Demonstrates the ability to provide expected patient contact as indicated by productivity reports and patient contact analysis.

Minimum Qualifications

Program Description
 

Putnam Court is a building which was developed and is operated by Dunn Development Corp., a private developer. Putnam Court is a newly constructed residential building located on Putnam Avenue between Irving Place and Downing Street in the Clinton Hill neighborhood of Brooklyn. The project consists of a seven-story elevator building containing 59 residential units, with 58 units of rental housing and one superintendent’s unit. CAMBA, Inc. is the on-site services provider at Putnam Court. Thirty-four apartments, 58% of the units in the building, will be master leased by CAMBA and will house Population A chronically homeless single adults who are diagnosed with SPMI (seriously and persistently mentally ill) or MICA (mentally III and chemically addicted). The remaining 24 units are occupied by community residents, who may require some services.

What The Case Manager Does:

  • Maintain professional relationships with clients and client confidentiality.
  • Practice Universal Precautions/Standard Protocol & Procedures.
  • Comply with all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.
  • Review all documentation establishing tenants' eligibility for program and make file copies.
  • Create and maintain tenant files.
  • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments.
  • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals.
  • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA.
  • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency.
  • Recommend and implement strategies to persuade tenants to participate more fully in this process.
  • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes.
  • Participate in case conferences.
  • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization.
  • Provide all required information for weekly/monthly/quarterly/annual reports.
  • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or (c) have become ineligible for services (e.g. moved out of building).
  • May act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.
  • May schedule appointments for tenants with referral organizations.
  • May escort tenants to appointments (educational, medical, social service, etc.)
  • May assist tenants in completing applications for benefits and entitlements and may process applications on tenants' behalf.
  • May follow up with tenants for a period after successful completion of their primary goals to assure tenant stability.
  • May evaluate actual living conditions of tenants through home visits.
  • May prepare marketing materials for the program.
  • May prescreen tenants over the telephone for eligibility and may schedule intake appointments.
  • May input tenant data and tenant progress information into automated database.
  • Tasks may be modified, expanded and/or assigned over time.

Education, Licenses and/or Certifications Required:

Bachelor's degree (e. g., B.A., B.S.W.) and experience working with the Seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience.

Preferred Qualifications:

  • As a Mandated Reporter, you must immediately report suspected incidents/cases of abuse and neglect to the Vulnerable Persons Center Register (VPCR) Hotline at 1-855-373-2122 and complete a CAMBA Incident Report.

Compensation: $42,000 - $45,000 annually

The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
