Your Role in the Mission As a Patient Care Manager, you lead a local hospice team with clarity, compassion, and clinical integrity. You guide care plans, coach team members, and oversee the delivery of services across nursing, psychosocial, spiritual, and support disciplines.

You are responsible for ensuring timely admissions, effective team communication, regulatory compliance, and the consistent delivery of safe, high-quality care. You model Lifted values in every interaction and help protect the unity, trust, and excellence that define the patient experience.

Key Responsibilities

Leadership & Team Coordination

  • Lead the interdisciplinary team Provide day-to-day guidance and oversight for nurses, social workers, chaplains, CNAs, and volunteers.
  • Support quality and continuity Ensure patient care is delivered in alignment with Lifted standards, hospice regulations, and ethical best practices.
  • Facilitate team meetings Serve as the team lead during IDT meetings, helping review and align care plans.
  • Mentor and coach staff Provide training, feedback, and performance coaching to clinical team members.
  • Support hiring and onboarding Assist with recruitment, orientation, and ongoing development of new staff.

Patient Care & Clinical Oversight

  • Complete assessments and admissions Conduct patient evaluations and initiate care plans as needed.
  • Monitor care plans Ensure care plans are regularly updated based on patient needs, preferences, and changes in condition.
  • Oversee clinical decisions Provide RN-level review and guidance during complex admissions or care changes.

Compliance & Quality Assurance

  • Monitor documentation Ensure accurate, timely charting in alignment with EMR standards and regulatory requirements.

Collaboration & Communication

  • Coordinate with providers Maintain regular communication with physicians, referral partners, and community agencies.

Technology & Documentation

  • Use EMR systems effectively Maintain accurate documentation in Kantime and support documentation accuracy across the team.
  • Navigate digital tools Utilize Google Workspace, Microsoft Office, and Lifted systems to support communication and operations.

Skills & Qualifications

  • Graduate of an accredited Diploma, Associate, or Bachelor's nursing program.
  • Active RN license in good standing, in accordance with state and/or Nurse Licensure Compact (NLC) requirements.
  • Current CPR certification (if required for direct patient visits).
  • Minimum of 2 years of hospice or home health nursing experience in the past 36 months; hospice strongly preferred.
  • Prior experience in clinical leadership or team supervision preferred.
  • Strong clinical judgment, assessment skills, and regulatory knowledge.
  • Excellent interpersonal and communication skills.
  • EMR proficiency required; KanTime experience preferred.
  • Strong organizational, mentoring, and time management abilities.

Where You'll Work

As a Patient Care Manager, your time will primarily be in the officesupporting your team, coordinating care, and ensuring clinical excellence across your assigned territory with some time spent in the field. You'll conduct visits as needed, attend IDT meetings, and collaborate with the full interdisciplinary team to ensure every patient receives timely, unified, and compassionate support.

About the Role

  • Conduct initial review of grant and contract proposals to ensure compliance with applicable University, Federal, State, etc. obligations and assurances.

  • Review information received from funding agencies and distribute to appropriate administrators and investigators; keep abreast of changes in policies and regulations of granting agencies.
  • Research and summarize information on government regulations pertinent to grants and contracts.
  • Coordinate the maintenance of a grants database, indirect cost policies, and subcontracts; code information for data entry in Banner Grants module.
  • Follow up with funding agencies and principal investigators concerning questions, inconsistencies or additional information/materials needed.
  • Compile data and prepare reports, set forth trends, analyze progress, and make recommendations or conclusions.
  • Coordinate the maintenance of the proposal and award filing system. Perform other related duties incidental to the work described herein.
  • Collaborate with grant accounting staff on the reconciliation of grant funds and audit inquiries.

Qualifications:

  • Bachelor's degree in Accounting, Finance, or Business required.
  • General knowledge of Federal research compliance regulations including OMB Circulars A-21, AllO, A- 113. Certification in Research Administration (CRA) is preferred. Or an equivalent combination of relevant education and/or experience.
  • At least one year of experience involving the administration or accounting of grants and contracts.
  • An equivalent combination of relevant education and/or experience will be evaluated.

The Position

The Help Desk Specialist will be responsible for maintaining operations oversight and situational awareness of the Continental United States (CONUS) Secret Internet Protocol Routed Network (SIPRNet) and Global Command and Control System (GCCS) Management Centers. Also responsible for providing Tier 1 and Tier 2 technical support for both GCCS and Joint Planning and Execution System (JPES), utilizing established customer-provided knowledge base, Standard Operating Procedures (SOP), ticketing system, and other system administration guidelines.


Responsibilities:

  • Respond to and diagnose UNIX/Linux problems through discussion with users and provide Tier 1 and 2 technical support by functioning as focal point for GCCS and JPES (emphasis on UNIX/Linux) and user problem resolution to include providing end-to-end ownership of incidents with actual or potential impact to operations and ensuring a timely process through which incidents/problems are controlled
  • Provide technical support for UNIX/Linux systems/applications within the SIPRNet and GCCS Management Centers to identifying, researching and resolving technical problems in a timely manner in response to customer incidents received via telephone calls, email and personnel requests with the intended goal to quickly resolve incidents on first contact
  • Utilize customer-provided knowledge base and ticket management system to enter, document, track, coordinate, route, resolve, and close user ticket issues to include creating and maintaining a central repository for technical advice and solutions for network systems, software applications assistance, automatic data processing support, hardware exchange, and repair service support, and assisting with monitoring and reporting performance metrics
  • Exercise sound judgement in developing methods, techniques, and evaluation criteria associated with obtaining results under this contract to include monitoring Key Performance Indicators (KPI) and recommending innovations to improve and enhance overall work center operations
  • Interact with technician across multiple disciplines to include, but not limited to networking, Tier 2 and 3, and application support, customers and contract leadership relating complex technical issues/solutions in a manner to establish effective partnerships/relationships in support of accomplishing the work required
  • Improve the customers operating environment by providing expert support while also maintaining the cybersecurity/IA posture of systems/hardware managed/administered for both contract and customers assets
  • Compliance with all safety and security requirements, policies, and procedures; worksite access protocols and updates, training requirements, required certifications, and other duties/projects as assigned
