Description:

SUMMARY:

Provides daily living support and assistance to individuals in one of the agency’s Community Integrated Living Arrangements under the supervision of the CILA Team Leader and with the support of the Supported Living Program Manager.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Provide training and guidance to the residents in Community Integrated Living Arrangement (CILA) in the areas of activities of daily living (example-hygiene), community integration (example-outings) and economic self sufficiency skills (example-accurate recording of financial transactions).
Provide direct personal care or oversight as needed.
Provide excellent customer service to residents and families and maintain positive relationships and lines of communication.
Ensures resident medication needs are met by monitoring and documenting medication regimen. Brings any health issues to the supervisor’s attention.
Works with and participates in the community in order to assist residents in utilizing resources and developing natural supports. Includes providing client transportation.
Completes all required documentation.
Prepares and/or utilizes basic household necessities such as food, program supplies, and cleaning supplies, and is responsible for the overall appearance of the home, including daily housekeeping tasks.
Follow and maintain all safety procedures and participate in safety drills.

Ability to interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to effectively present information in one-on-one and small group situations to residents and other employees of the organization.

MATHEMATICAL SKILLS:

Ability to add and subtract two digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume and distance. Ability to compute percentages.

REASONING ABILITY:

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

An Endodontist is a dental specialist who focuses on the diagnosis, prevention, and treatment of diseases and injuries affecting the dental pulp and tissues surrounding the roots of the teeth. They specialize in performing root canal therapy and other endodontic procedures to save natural teeth and alleviate pain caused by dental infections or trauma.

*Duties and Responsibilities:*
The duties include, but are not limited to:

* Diagnosis and Treatment Planning:
* Conduct comprehensive examinations and diagnostic tests to identify dental pulp and periapical diseases, including pulpitis, apical periodontitis, and dental abscesses.
* Interpret radiographic images, such as X-rays and CBCT scans, to assess the extent of dental pathology and plan appropriate treatment interventions.
* Develop individualized treatment plans based on the diagnosis, considering factors such as the severity of the infection, the patient’s oral health status, and treatment preferences.
* Root Canal Therapy:
* Perform root canal procedures to remove infected or inflamed dental pulp from the pulp chamber and root canals of affected teeth.
* Clean and shape the root canal system using specialized instruments and irrigation solutions to eliminate bacteria and debris
* Fill and seal the root canal space with biocompatible materials, such as gutta-percha and sealer, to prevent recontamination and promote healing of periapical tissues
* Endodontic Surgery:
* Provide surgical endodontic treatments, including apicoectomy (root-end resection) and root-end filling, to address persistent or recurrent periapical lesions
* Perform surgical interventions to repair root fractures, remove foreign objects from root canals, and treat other complex endodontic conditions
* Traumatic Dental Injuries:
* Manage traumatic dental injuries, such as avulsions, luxations, and fractures, by assessing the extent of damage to the dental pulp and supporting structures
* Perform vital pulp therapy, apexogenesis, or apexification procedures to preserve pulp vitality and promote continued root development in immature teeth following traumatic injuries
* Pain Management and Emergency:
* Provide prompt and effective pain relief for patients experiencing dental emergencies related to pulpits, periapical abscesses, or traumatic dental injuries
* Administer local anesthesia and analgesics as needed to ensure patient comfort during endodontic procedures and post-operative recovery
* Patient Education and Counseling:
* Educate patients about the importance of maintaining optimal oral hygiene practices and attending regular dental check-ups to prevent dental infections and preserve oral health
* Discuss treatment options, risks, and benefits with patients and address any concerns or questions they may have regarding endodontic procedures
* Collaboration and Referrals:
* Collaborate with general dentists, prosthodontists, and other dental specialists to develop comprehensive treatment plans for patients requiring multidisciplinary care
* Refer patients to specialists in related fields, such as oral surgeons or periodontists, for advanced treatments or procedures outside the scope of endodontics
* Perform miscellaneous job-related duties as assigned.

*Qualifications:*

* Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) degree from an accredited dental school
* Completion of an accredited endodontic residency program
* Board certification or eligibility for board certification in endodontics
* State licensure to practice endodontics
* Excellent clinical skills, including proficiency in performing root canal therapy and endodontic surgery.
* Active CPR Certification is required

Fountain House’s Community Oriented Recovery and Empowerment (CORE) services is a program serving individuals living with severe and persistent mental illness. This program serves Medicaid HARP enrollees and works to assist individuals in reaching their recovery goals. Services provided include Psychosocial Rehabilitation, which incorporate education and employment goals, and Peer Support. There is an extensive and broad range of services that include but are not limited to the improvement of daily living skills, managing finances, obtaining or improving safe and affordable housing, furthering one’s education, obtaining employment, achieving personal recovery and/or resilience, improving family and interpersonal relationships, preventing social isolation, connecting with effective and comprehensive behavioral health and primary care, and full community integration. The CORE Services Certified Peer Specialist is responsible for ensuring members receive person-centered, trauma-informed and culturally competent services with a rehabilitation and recovery focus in their homes or their community. Direct Service Responsibilities

Help individuals work on goals such as community connectedness, development of natural resources, physical and mental health wellness, education, employment, self-help, and independent living skills.

Assist with and coordinate transportation to appointments, including provision of travel training as needed.
Accompany members to behavioral health, medical and/or other appointments to support and facilitate engagement and follow through as needed.
Coordinate and communicate care planning with other providers of services.
Attend and participate in collaborative CORE team meetings.
Act as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services.
Maintain compliance with all New York State Office of Mental Health policies and procedures.
Maintain organized and accurate documents and client records using AWARDS - electronic health record (EHR) database.

What you'll do as a CDL-A Delivery Driver:

• Inspect tractor-trailer for defects pre/post trip and submit DOT inspection report indicating condition.
• Inspect bill of lading and store keys for accuracy in off-hour delivery.
• Drive tractor-trailer to destination, applying knowledge of commercial driving regulations and skill in maneuvering vehicle on the road and on customer premises.
• Maintain driver log (Manual or Peoplenet) according to DOT regulations, documenting delivery receipt, product temperatures and exceptions.
• Unload trailer, delivering product into customer premises.
• Other duties as assigned.

Qualifications you'll bring as a CDL-A Teammate:

• At least 21 years of age
• Valid Class A commercial driver's license (CDL-A)
• At least 1 year or 50,000 verifiable miles of tractor-trailer driving experience
• Must meet McLane's MVR and risk rating qualifications
• This position requires the ability to read, write, and understand English at a level sufficient to perform job-related tasks effectively and safely. This includes understanding work instructions, safety protocols, and communications essential to the role. The requirement is directly related to the nature of the job and ensures compliance with workplace safety and operational standards.
